Waldorf Center Financial Policies

 
  financing As patients approach surgery, they frequently need instructions about the payment arrangements and have questions about their potential insurance benefits. We hope the following information will be helpful.

  Our Patient Relations staff are available to meet with you personally to provide the specific information you desire. They specialize in this area and will use their expertise to help you obtain the maximum benefits from your policy.

  Payment Policy

 

Payment for cosmetic plastic surgery is due in full at the time of
your preoperative visit. The total amount due will be paid to three
separate parties: The Waldorf Center for Plastic Surgery, for your physician fees; West Hills Surgery Center, for your facility fees; and, Oregon Anesthesia Group (OAG) for your anesthesia fees. All fees for cosmetic procedures must be paid prior to surgery.


CASH OR CHECK Personal check, cashier’s check, or cash.

CREDIT CARDS Visa, Mastercard, American Express, or Discover

 
FINANCING IS AVAILABLE for any procedures our staff offers
between the amounts $800-$20,000. If you would like more
information and an application please ask our staff.

NOTE: Dr. Waldorf and Dr. Jewett have signed agreements with insurance companies that legally, we cannot bill for cosmetic procedures.
Therefore, under no circumstances will insurance companies be
billed for cosmetic procedures.

  Appointment Policy

 

We understand that a situation may arise that could force you to
postpone your surgery. Please understand that such changes affect
not only your surgeon but other patients as well. Dr. Waldorf and Dr. Jewett's
time, as well as that of the operating room staff, is valuable and
we request your courtesy and concern.

If you need to cancel your surgery after your preoperative visit
but more that 10 business days before surgery, you are entitled to
a full refund. Cancellations 1-9 days prior to surgery will be charged $200, unless we are able to fill your cancelled appointment..


 

If you have any questions or need assistance with financial
matters, please speak with our business manager.

  Revision Policy

  insurance plans If revisions are necessary within the first 6 months the surgeon's fee may be waived; however, the cost of the facility, supplies, and anesthesia would be your responsibility.

  Insurance Plans

  We accept most insurance plans for non-cosmetic services. It is your responsibility to contact your insurance company, prior to your appointment, to ensure that your visit will be covered and that all necessary referrals are in place. Bring your insurance card to your appointment and we will bill your
plan for you.
